Affidavit of Loss (sample)

I never thought that losing something like your company ID might be a headache in the process. You need to submit a formal statement stating the loss of the said thing. And to submit an AFFIDAVIT of LOSS is definitely a requirement. It was too unfortunate that our company is issuing new proximity IDs and we need to surrender our old ones. I have nothing to do but to secure an AFFIDAVIT of LOSS and pay Php 300 for the replacement. Learning that I need to, I researched online for samples of such affidavit and what other requirements it might need. Thanks to the help of my office mates who gave me an idea on how to make one.

The requirements:


1. Get a sample of it (see above):
2. Secure a Community Tax Certificate (CTC)- you can go to your nearest city hall or barangay hall. In my case, I didn’t get one! Sssshhhhh…
3. Have the affidavit notarized. Go to any law firm or seek an attorney and inquire their notary public fees. It may range from Php 100-250 (that’s what I know).

It cost much on my part as it seems too legal to do such. Well, as a good citizen of the country. I must! I gave a call to our neighbor attorney and inquired on the notary public fee, he would charge mePhp 200! WTF!!! Just for his signature?! Duh, what’s the matter with him? Well, I don’t give him a damn thing, I seek another option. Good thing, my brilliant mates brought me to Colon here in Cebu, somewhere along the sidewalk beside the Sto. Nino church, I got my affidavit of loss notarized by just Php 30! Hahahaha…
I didn’t get any CTC, take note!

Just today, I went to our Security Office to submit the notarized form and claimed my new ID. Voila! In minutes, I have it!
